Hemorrhoid creams that contain phenylephrine are often advertised based on their cooling effects. Dr. Shereene Idriss, a New York City-based dermatologist, recently called out these dangerous practices in a viral TikTok video. She noted that using these creams can “worsen the look of your wrinkles and the redness around your eyes.”

Dr. Mark Strom, another NYC dermatologist, reinforced these warnings in a separate TikTok video. He warned consumers to avoid them in particular with hemorrhoid creams with hydrocortisone. The problem is that this common topical steroid thins the delicate skin around the eyelids when used long-term.

Phenylephrine works by constricting the blood vessels under your skin, which can temporarily minimize puffiness. If you use it close to your eyes, it can lead to significant health issues. “This ingredient can increase the risk of glaucoma when used in conjunction with steroids around the eyes,” warned Dr. Strom. Glaucoma is a progressive eye disease that can cause irreversible blindness without timely treatment.
